Gastric Bypass Surgery is demonstrated by Dr. Brett Cohen of the Memorial Weight-Loss Surgery Program. During gastric bypass, a small pouch is cut out of the stomach, and the small intestine is rerouted to this pouch. This tends to be a quicker, more aggressive weight-loss option. Unlike the Lap Band, it is neither adjustable, nor reversible. This type of surgery typically involves a about a two-night hospital stay. Although most of the stomach and the upper part of the small intestine no longer have food traveling through them, they still produce important digestive juices that are emptied into the small intestine and help with digestion. Most people who have gastric bypass experience a significant reduction in excess weight within the first 18 months after surgery. Your digestive system or gastrointestinal tract includes your mouth, esophagus, stomach, small intestine, and large intestine. Your pancreas, liver, and gallbladder squirt digestive juices into your small intestine to help you break down the food you eat. Your doctor may recommend bariatric surgery if you are unable to lose weight and keep it off through diet and exercise alone, have a body mass index or BMI above 40, have a BMI above 35 and you have a life threatening condition such as heart disease or diabetes, or weigh more than 100 pounds over your ideal body weight or IBW. Bariatric surgery helps you lose weight by decreasing the size of your stomach so that it holds less food and makes you feel full more quickly. Before your procedure, an intravenous line or IV will be started. You may be given antibiotics through the IV to decrease your chance of infection. You'll be given general anesthesia. A breathing tube will be inserted through your mouth and down your throat to help you breathe during the operation. Your surgeon will make a small incision near your belly button and insert a plastic tube called a port. Carbon dioxide gas will be pumped into your abdomen through this port. The gas will inflate your abdomen, giving your surgeon more room to see and move the surgical tools. After your abdomen is inflated, a high definition camera will be inserted into this port. Your surgeon will make additional port incisions for robotic instruments, as well as for instruments used by patient side assistance. An assistant will insert all of the robotic tools through these ports. Unlike standard laparoscopic instruments, these tools can rotate 360 degrees and have more flexibility than the human wrist. Seated at a special console, your surgeon will operate the robotic arms and the camera with joystick-like controls and foot pedals. A computer will translate the exact movements of your surgeon's fingers into precise movements of the surgical tools. At the same time, a high definition vision system will provide a magnified, three dimensional stereoscopic view of the surgical area. One common robotic bariatric surgical procedure is a gastric sleeve procedure. In this procedure, most of your stomach will be removed, leaving a smaller sleeve-like tube about the size of a banana. To create the gastric sleeve, your surgeon will cut and staple your stomach at the same time. The part of your stomach separated from the gastric sleeve will be removed through one of the incisions. The remaining gastric sleeve does not change where food enters and leaves your stomach. At the end of your procedure, the incisions will be closed with stitches, staples, surgical glue, or closure tape dressings. The incisions will be covered with sterile dressings. After your procedure, your breathing tube will be removed, and you will be taken to the recovery area for monitoring. You'll be given pain medication as needed. You may continue to receive antibiotics through your IV. Most patients are released from the hospital one to two days after the procedure. ANR17010

In this procedure a cut is made vertically and the larger part of the stomach - up to 85% of its volume - is removed from the body. The remainder is closed with staples to create a new stomach that is now in the shape of a tube. The procedure preserves both sphincters at the two ends of the stomach and therefore has minimum effect on the functioning of the digestive process. This reduces the risk of malabsorptive complications commonly associated with intestinal bypass. The procedure is irreversible. Vertical sleeve gastrectomy procedure is gaining more and more popularity thanks to its simplicity and good results on initial weight loss. However, due to the lack of long-term data, it is yet to be endorsed by bariatric surgery societies and is not covered by some insurance companies. Adjustable gastric band or Lap band, advantages and disadvantages. In this procedure an inflatable silicon band is placed around the top of the stomach to create a small stomach pouch. The band is connected to a port placed under the skin of the abdomen. Through this port, a saline solution is injected to adjust the diameter of the band and thus making the passage between the pouch and the lower part of the stomach smaller or larger according to the needs of the patient. During a meal, the pouch is filled up quickly with a small amount of food and releases it slowly into the lower part of the stomach due to the restriction by the band. As the pouch is full, it gives the feeling of satiety or fullness and helps to stop the patient from further eating. Slow passage of the food makes the patient feel full for a longer period of time and thus reduces the amount of food intake. As the procedure involves no cutting or stapling of the stomach, it is minimally invasive and reversible. The surgery can be performed laparoscopically through small incisions with the aid of a camera. Recovery time is significantly shortened compared to gastric bypass procedures. Also, as there is no intestinal bypass, the risks of nutritional deficiencies, dumping syndrome and other complications associated with it are significantly reduced. In terms of weight loss efficiency, however, patients who undergo gastric lap band surgery typically lose less weight than those who have had gastric bypass procedures.

Dr. Jason Fung, MD, is a nephrologist and expert in the use of intermittent fasting and low-carbohydrate diets for the treatment of Type 2 diabetes. In this presentation, delivered on Aug. 2, 2018, at the 2018 CrossFit Health Conference in Madison, Wisconsin, Fung shares his first-hand experiences with “The Mess” and discusses how he shifted his research and medical practices as a result of those experiences. Fung’s objectives for his presentation include: 1. Understanding why long-term weight loss is so difficult. 2. Introducing the concept of therapeutic fasting. 3. Understanding some myths and misunderstandings associated with the fasting process. He recalls treating obese and diabetic patients with traditional methods, which included what he characterizes as poor dietary recommendations and a slurry of drugs. He explains, “It became obvious that I’m just sort of holding their hand until they get their heart attack, until they get dialysis, until they go blind, until we chop their feet off.” “It’s really sad to realize that the profession that you’ve chosen is not really helping people,” he says. This realization compelled him to diagnose the problems associated with traditional care and seek alternative treatment methods for his patients. Fung historicizes what he calls “the modern eating pattern,” which emerged in 1977 in the U.S. with the development of the Dietary Guidelines for Americans. He notes the 1977 guidelines led to the consumption of more grains and sugars, which in turn led to people “eating often, eating late, and eating all the time.” Incidences of obesity and Type 2 diabetes reached epidemic levels, and the most common treatments long have been drug interventions. Unfortunately, the prevailing non-pharmaceutical prescription — to eat less and move more — has a 99.9% failure rate. Fung observes that popular wisdom tells us to blame the patient and assume he or she did not adhere to the prescription. He claims a basic understanding of metabolism suggests otherwise, however. Fung explains why a significant reduction in caloric intake leads to a decrease in basal metabolism. This biological inevitability is ignored by the proponents of the “calories in, calories out” fallacy, he observes. He also explains why intermittent fasting is an effective alternative to traditional treatments for obesity and diabetes. The modern eating pattern keeps our insulin levels high all the time as we eat over long durations, and when insulin remains high all the time, Fung explains, our bodies store food energy as fat, and we remain hungry. Intermittent fasting, on the other hand, allows insulin levels to drop, which puts us in burning mode rather than storing mode. Fung claims his recommendations are so effective that patients no longer need to say, “Oh wow, I have to go see my doctor to see what pill I need,” or, “I need to go see my doctor to see if he needs to stick a stent in me.” Instead, Fung explains, “We’re giving you the power to take back your own health, because you’re not gonna get it from anywhere else.” CrossFit® - Forging Elite Fitness® (🤍
